The form was prepared by the Michigan Corporation & Securities Commission and was completed by the applicant in black typewritten text (carbon copy). It has been signed by Captain Morgan Howell, Captain Frank Becker, Frank N. MacLean, and Robert A. Zeleznik, and was dated May 25, 1950. Blue ink stamps on the front indicate that the form was received by the Michigan Corporation & Securities Commission on May 29, 1950, and was filed on May 31, 1950.
