Ballot of candidate for initiation and permanent visiting membership to the Palestine Lodge No. 357 of the Free and Accepted Masons. Each candidate's name, age, occupation, business address, home address, are listed along with who recommended them and to whom they have been referred. The candidates are invited to visit the lodge and dine there on May 17, and the vote occurs on June 7, 1912. The ballot is printed on yellow paper that has been folded three times.
