One "Official All-Star Election - Major League Baseball" ballot. The punch card ballot is printed in red and blue ink on both sides of a rectangular piece of heavyweight white paper. The front of the card shows instructions in blue printed text, a baseball logo, and blue printed text that is addressed to "The Commissioner of Baseball, All-Star election Hdqrs., P.O. Box 1, New York, N.Y. 10046." Adjacent blue handwritten text shows the signature of "Sonny Eliot" and the date, "5/28/75."
The back of the card is printed blue text which lists the names of American League and National League baseball players who were All-Star candidates. A small pre-cut rectangle is located next to each player name. Ballot selections were to be made by punching out the rectangle adjacent to the selected player name. Blank lines at the bottom of the card were available for write-in selections.
