Each of the four coupons on the surviving sheet is valued at ten cents toward a ride in a Checker Cab Company taxi cab. The cover is printed with a green on pink checker pattern, and the coupons within are pink. The cover and the coupons list the company's phone number as WOodward 3-7000. The signature of Checker president Alfred Diaz is printed on each coupon.
