It is printed with the signature of Mayor John C. Lodge. The space for the signature of the organization's director is blank. A blue embossed seal, likely for the City of Detroit Department of Health, but since worn down has been affixed to the lower left corner. "1928- Shop & Res, 10206 Prairie Ave." is handwritten on the verso.
