One form, "Appointment of Resident Agent," that was completed by Frank Becker and Robert A. Zeleznik of the International Tugboat Racing Association and was later filed with the Michigan Corporation and Securities Commission. The pre-printed form has black text on white paper with blank spaces that have been completed in black typewritten text. It was signed and dated May 25, 1950, but was not notarized until September 18, 1951.
