The form consists of eight rows to track the names, telephone numbers, addresses, and remarks of individuals who were mailed the promotional folders. Text at the bottom of the form reads "These people must be called within 3 days after the folder is mailed and a report written under each name. Return this sheet to the Branch Manager."
